St Simeon Healthcare is a trusted provider of aged care services committed to supporting older people to live safely, independently, and with dignity in their own homes. We are dedicated to delivering high-quality, person-centred services that uphold the rights, choices, and wellbeing of older people in accordance with the Aged Care Act 2024, Support at Home Program requirements, and Aged Care Quality Standards.
We are seeking an experienced and motivated Case Manager to join our Home Care team on a full time ongoing basis.
This role is responsible for overseeing and managing the care of Support at Home clients across the Sydney metropolitan area, primarily within Western Sydney and the Nepean region. The successful applicant will play a key role in supporting older people to achieve their goals, maintain independence, and access services that enhance their quality of life.
About the Role
As a Case Manager, you will be responsible for supporting older people throughout their aged care journey, from intake and onboarding through to ongoing care planning, service coordination, monitoring, and review.
You will work closely with clients, their families, carers, representatives, healthcare professionals, and service providers to ensure services are responsive, person-centred, and aligned with each client's goals, preferences, assessed needs, and available funding.
This position requires excellent communication, organisational, and problem-solving skills, together with a strong understanding of contemporary aged care practices, including wellness, reablement, consumer choice, dignity of risk, and supported decision-making principles.
Regular travel throughout the Sydney metropolitan area is required.
Key Responsibilities
Conduct client assessments and develop comprehensive,
person-centred care plans that reflect individual goals, strengths, preferences, cultural needs, risks, and support requirements.
Support older people to exercise choice and control over the services they receive while promoting independence, wellbeing, and quality of life.
Apply wellness and reablement principles to assist clients in maintaining or improving their functional capacity and independence.
Coordinate and monitor services in accordance with client needs, goals, approved funding, and program requirements.
Monitor client budgets, service utilisation, and expenditure to ensure services remain sustainable and aligned with available funding.
Conduct regular care reviews and reassessments to evaluate outcomes and respond to changing needs.
Foster positive working relationships with clients, families, carers, registered supporters, representatives, and multidisciplinary teams.
Collaborate with healthcare professionals and service providers to coordinate effective and timely service delivery.
Complete risk assessments and identify safeguarding concerns, including abuse, neglect, self-neglect, and other risks to client wellbeing.
Escalate incidents, complaints, and identified risks in accordance with organisational policies and legislative requirements.
Maintain accurate, timely, and confidential client records and documentation.
Contribute to quality improvement activities, audits, and compliance processes.
Ensure all services and practices comply with the Aged Care Act 2024,
Support at Home Program requirements, Aged Care Quality Standards, privacy legislation, and organisational policies and procedures.
What You'll Bring
Bachelor’s degree or Diploma in Community Services, Human Services, Social Work, Nursing, Allied Health, Case Management, Service Coordination, or a related discipline.
Demonstrated experience in Aged Care, Home Care, or a related sector.
Experience undertaking client assessments, care planning, service coordination, and case management.
Strong understanding of person-centred care, wellness and reablement approaches.
Knowledge of the Aged Care Act 2024, Support at Home Program, and Aged Care Quality Standards.
Ability to identify and respond appropriately to risks, incidents, abuse, neglect, and safeguarding concerns.
Excellent communication, relationship-building, organisational, and time management skills.
Ability to work independently while contributing effectively within a multidisciplinary team environment.
Advanced computer skills, including experience using client management systems, email, and mobile applications.
Current Australian Driver Licence and access to a reliable motor vehicle with Full Insurance.
Current National Police Check and eligibility to work in aged care.
Experience working within Home Care Packages and/or Support at Home programs.
Enrolled Nurse (EN) or Registered Nurse (RN) qualifications are highly regarded.
Experience using Manad care management and client management software systems.
Understanding of culturally safe and inclusive practices.
Ability to speak an additional language.
