About the role

Aequra Residences is seeking an experienced Clinical Care Coordinator to support clinical quality, staff capability and regulatory compliance at our Punchbowl residence. Reporting to and working closely with the Care Manager, the Clinical Care Coordinator will primarily coordinate staff education, competency assessment, clinical auditing and continuous improvement activities.

The role will analyse audit results, incidents, clinical data, complaints and feedback to identify improvement opportunities and support the Care Manager and care team to implement and monitor corrective actions.

The Clinical Care Coordinator will also assist the Care Manager with clinical oversight, care planning and other clinical responsibilities when required, including during periods of leave or increased operational demand.

The successful candidate will be an experienced Registered Nurse with strong knowledge and demonstrated practical application of the Aged Care Act 2024, the strengthened Aged Care Quality Standards and Aged Care Quality and Safety Commission requirements.

KEY RESPONSIBILITIES

Education and staff capability

Assess the education, training and competency needs of clinical and care employees.

Develop, coordinate and deliver education programs aligned with identified risks, audit findings, incidents, complaints and regulatory requirements.

Coordinate clinical orientation and induction for new clinical and care employees.

Conduct and document competency assessments within the employee’s role and scope of practice.

Maintain accurate education, attendance, competency and professional-development records.

Monitor mandatory training compliance and follow up outstanding requirements.

Provide coaching and practical support to employees to strengthen clinical practice and documentation.





Evaluate the effectiveness of education programs and identify areas requiring further support.

Auditing and compliance

Conduct audits of clinical documentation, care plans, assessments, medication management, infection prevention and control, incident management, restrictive practices and other relevant care processes.

Analyse audit findings, identify trends and prepare clear reports and recommendations for the Care Manager and management team.

Support the development, implementation and monitoring of corrective-action and continuous-improvement plans.

Monitor whether agreed corrective actions have been completed and are achieving the intended outcomes.

Assist with preparations for Aged Care Quality and Safety Commission audits, assessment contacts and other regulatory activities.

Support compliance with the Aged Care Act 2024, the strengthened Aged Care Quality Standards, the Aged Care Rules 2025 and organisational policies and procedures.

Assist with maintaining evidence demonstrating the residence’s compliance with applicable regulatory requirements.

Quality improvement and clinical data

Review incidents, complaints, feedback, clinical indicators and audit results to identify trends, risks and opportunities for improvement.

Support the investigation and review of clinical incidents and contribute to the identification of corrective and preventive actions.

Coordinate and monitor relevant continuous-improvement initiatives.

Prepare clinical quality reports and present findings to the Care Manager and management team.

Support the implementation and evaluation of new clinical procedures, systems and improvement initiatives.

Promote a culture of learning, accountability, open disclosure and continuous improvement.

Support to the Care Manager





Assist the Care Manager with clinical oversight and resident-care matters as required.

Support reviews of care plans, assessments and clinical documentation where risks, gaps or improvement opportunities are identified.

Provide guidance to clinical and care employees in consultation with the Care Manager.

Assist with the monitoring and follow-up of identified clinical risks.

Support communication with residents, families, representatives and health professionals when delegated by the Care Manager.

Provide appropriate clinical support during periods of increased operational demand or Care Manager leave.

Escalate significant clinical, compliance or workforce concerns promptly to the Care Manager or Residential Manager.

QUALIFICATIONS AND EXPERIENCE

Current unrestricted registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency.

A minimum of five years’ experience as a Registered Nurse in residential aged care.

At least three years’ experience in a Clinical Care Coordinator role or a comparable position with substantial responsibility for clinical education, auditing and continuous improvement.

Strong knowledge and demonstrated practical application of the Aged Care Act 2024, the strengthened Aged Care Quality Standards and Aged Care Quality and Safety Commission requirements.

Demonstrated experience developing and delivering education and competency-assessment programs for clinical and care employees.

Demonstrated experience conducting clinical audits, analysing findings and monitoring corrective actions.

Ability to work collaboratively with and provide effective support to the Care Manager and broader management team.

Strong digital literacy and experience using electronic care-management and learning-management systems.

Current National Police Certificate and capacity to satisfy all applicable aged-care worker screening requirements.

Qualifications in workplace training and assessment, clinical education, aged care, infection prevention and control or quality management.

Experience providing clinical support in a complex residential aged care setting.
