About the Role
We’re looking for an experienced and compassionate Care Partner to join us,
working across home and community settings to support our growth in the Southeast region, Sydney.
This is a energetic, field-based role – not a desk job. You’ll be out in the community, supporting clients in their homes and local settings, delivering high-quality, person-centred coordination and care.
Your portfolio will focus on clients funded under the NDIS, including people living with disability and those recovering from injury under iCare NSW ensuring support is personalised, respectful and aligned with funding requirements.
What You'll Be Doing
As a Care Partner,you’llplay a key role in:
- Conducting holistic assessments covering physical, psychosocial,culturaland environmental needs
- Developing,implementingand reviewingindividualisedcare plans built around each client’s goals, wellness,safetyand quality of life
- Being the primary point of contact for clients, families and carers and advocating for their rights,dignityand preferences
- Running regular check-ins, progressreviewsand site audits tomaintainquality,safetyand satisfaction
- Monitoring client notes,incidentsand service trends to spot risks early and escalate appropriately
- Reporting accurately and on time to agencies (NDIA,iCare)NSW in line with program standards
- Growing your client portfolio by sharing the Leora story with clients,representativesand industry partners
About You
To succeed in this role, you’ll bring:
- A qualification in allied health, aged care,disabilityor a related community field
- Community engagement experience, including networking and stakeholder engagement
- Strong stakeholder management and relationship-building skills
- Excellent verbal, written and presentation communication
- Strongorganisationand decision-making skills, especially in fast-paced environments
- A valid Australian DriverLicence(community travel is essential)
- A genuine commitment to improving the lives of people living with disability
You’llalso need a valid NDIS Worker Screening Check (or willingness to obtain one).
