Job tasks and responsibilities

- Assess suitability, capacity and function to inform interventions using a wellness and reablement approach to prescribe, deliver, adapt, and evaluate exercise-based interventions that enhance and maintain function and quality of life.
- Develop goal-oriented care plans and individualised interventions, in partnership with the consumer to facilitate recovery or rehabilitation.
- Contribute to a multidisciplinary team approach to deliver holistic programs.
- Deliver individual and group services in a variety of environments including home, community and clinic onsite.
- Liaise with clients, referral agencies, medical teams, service providers, family/advocate to ensure best possible outcomes.
- Provide health education and advice to empower people to improve health outcomes and self-manage health conditions.
- Coach and motivate to increase engagement in exercise, including addressing client preferences, needs, barriers, and goals.
- Apply a person-centred approach to people of diverse backgrounds and populations and work collaboratively with clients and relevant others involved in their support.




- Practice in a culturally safe and inclusive manner according to the principles of person-centred care and apply appropriate in-person and digital practices such as telehealth.
- Current registration with ESSA as an accredited Exercise Physiologist
- Current motor vehicle driver's licence
- Two (2) professional referee checks - worked with candidate recently
- National Police Certificate obtained within the last three (3) months
- Strong interpersonal and verbal/written communication skills enabling effective interaction with consumers and their families, team members, external health professionals and key stakeholders.
- Comprehensive understanding of community-based service provision.
- Demonstrated computer literacy with experience in word processing, calendar management software (Outlook) and data base applications.
- An understanding of risk management principles, workplace health and safety and continuous quality improvement processes and their application in the workplace.
- Desirablepostgraduate experience in providing evidence based clinical assessment and planning appropriate interventions for consumers with chronic and/or complex conditions, with a focus on consumer reablement and independence using a wellness/restorative approach to service provision.
- The ability to work autonomously and as a member of a multidisciplinary team.
