About the Role

As a Case Manager, you’ll deliver person-centred, high-quality care by managing a portfolio of clients and developing care plans that align with individual needs, funding and clinical requirements. You’ll build strong relationships, monitor client outcomes, and ensure services are safe, responsive and delivered to a high standard.

Key Responsibilities

- Manage an allocated client portfolio, ensuring services are planned, delivered and reviewed in line with client goals and funding requirements




- Conduct structured assessments and develop tailored care plans reflecting individual needs, risks and preferences
- Coordinate end-to-end client onboarding, ensuring timely intake and smooth transition into service delivery
- Monitor client funding utilisation, identifying underspend or overspend and adjusting to maintain financial sustainability
- Build effective relationships with clients, families, funding bodies and support coordinators
- Manage incidents and complaints in accordance with organisational frameworks
- Maintain accurate, audit-ready documentation and identify opportunities to improve service delivery

About You

You are an experienced, client‑focused skilled who enjoys building relationships and solving problems. You bring strong organisational skills, sound judgement and a genuine commitment to helping clients achieve their goals.

To Be Successful You Will Have

- Tertiary qualification in health, community services, nursing or a related discipline (or equivalent experience)
- Current National Police Clearance (or willingness to obtain)
- Working With Children Check, where applicable to the role
- NDIS Worker Screening Clearance (or willingness to obtain)
- A valid driver’s licence
- Evidence of current mandatory immunisations
- Full right to work in Australia
