About the role
We are seeking an NDIS Growth and Support Coordinator to join our team at Relative Care team in NSW on a fulltime basis. This is a strategic and essential position within our organisation, by becoming a custodian of client planning but also growing the organisation in a sustainable way. You will maintaining a cohort of support coordination clients whilst growing the scope of business to where the organisation may bring further resources for you to manage. As an NDIS Coordinator, you will be the vital link between our participants, their families, support workers and the NDIS system, ensuring seamless service delivery and optimal outcomes for all those in our care. This role plays a crucial part in maintaining our commitment to providing high-quality, person-centred disability support services across the region.
Key responsibilities
Business Development & Growth
- Develop and implement business growth strategies to expand the organisational services across NSW.
- Support with gaining participant enquiries and referrals from initial contact through to service commencement.
- Have and build upon and maintain strong relationships with other Support Coordinators, Local Area Coordinators (LACs), hospitals, allied health professionals, discharge planners, community organisations and other referral partners.
- Represent the organisation at networking events, expos, stakeholder meetings and community forums.
- Monitor referral pipelines and generate regular business development and performance reports.
- Identify new opportunities for service expansion and strategic partnerships.
- Understands NDIS legislation, price guide, and funding categories.
- Accurately interprets participant plans, goals, and budget allocations.
- Can explain plan components clearly to participants and families.
- Identifies risks, gaps, or misaligned supports within a plan.
- Manage scheduling or service delivery and resolve any issues
- Demonstrates a solid understanding of how plan transitions between providers work and confidently manages the full end-to end process to offboard from an existing provider and onboard to a new one.
Support Connection & Coordination
- Sources appropriate providers based on needs, goals, and preferences.
- Facilitates service agreements and bookings confidently.
- Ensures providers understand participant needs and expectations.
- Coordinates multidisciplinary teams effectively (allied health, support workers, SIL, etc.).
- Ensure the correct support workers are aligned to the participants needs.
- Adjust support as needs change
Plan Monitoring & Budget Tracking
- Monitors spending across Core, Capacity Building, and Capital budgets.
- Identifies underspend/overspend early and adjusts supports accordingly.
- Tracks provider performance and service delivery quality.
- Uses dashboards or tools to maintain accurate utilisation records.
- Monitor support and plan usage by tracking spending across budgets to avoid underspend or overspend.
- Identifies early signs of risk (health, behavioural, housing, social).
- Responds appropriately to crises or urgent changes in circumstances.
- Resolves provider conflicts or service delivery issues professionally.
- Escalates concerns appropriately (NDIA, clinical teams, safeguarding bodies).
Documentation & Reporting
- Maintains accurate, timely case notes aligned with NDIS standards.
- Prepares evidence for plan reviews or change‑of‑circumstances requests.
- Documents participant progress, barriers, and outcomes clearly.
- Ensures all records meet compliance and audit requirements.
- Gathers reports from providers ahead of review meetings.
- Assesses goal progress with the participant.
- Provides recommendations for future funding based on evidence.
- Supports participants during review meetings if requested.
- Works effectively with families, carers, providers, and stakeholders.
- Advocates for participant rights, preferences, and safety.
- Ensures culturally safe and trauma‑informed practice.
- Maintains professional relationships and manages boundaries well.
Professional Conduct & Compliance
- Upholds NDIS Code of Conduct and safeguarding principles.
- Demonstrates ethical decision‑making and confidentiality.
- Maintains up‑to‑date knowledge of NDIS changes and best practice.
- Completes mandatory training and organisational compliance tasks.
What we're looking for
- Proven experience working within the NDIS sector or disability support services
- Sound knowledge of the NDIS framework, participant planning processes and funding mechanisms
- Strong organisational and administrative skills with the ability to manage multiple participant cases simultaneously
- Excellent communication skills with the ability to engage effectively with participants, families, staff and external stakeholders
- Proficiency with computer systems and databases, particularly NDIS-related software platforms
- Demonstrated commitment to person-centred care and supporting individuals with disabilities
- Ability to work independently and as part of a collaborative team
- Strong attention to detail and accuracy in record-keeping and documentation
- Current Working With Children Check (or willingness to obtain) and a valid police clearance
- Relevant qualifications in community services, social work, disability services or related field (preferred)
- Right to work in Australia
- Reliable insured vehicle
- Current Australian Driver Licence
