About the opportunity

As the Home Support Assessor you will be supporting the delivery of the Footprints Aged Care Assessment Service (ACAS) by conducting Home Support Assessments for older Australians seeking access to Commonwealth-funded aged care services through the My Aged Care Gateway, and identifying when a client's needs extend beyond the scope of a Home Support Assessment.

The Home Support Assessor is responsible for gathering information from clients, carers, representatives, and other relevant stakeholders to develop a comprehensive understanding of the individual's circumstances and support needs. The role applies evidence-informed assessment practices to develop person-centred support plans, facilitate referrals, maintain accurate documentation, and manage an allocated caseload in accordance with legislative, contractual, quality, and organisational requirements.

Working collaboratively with clients, carers, service providers, healthcare professionals, and internal stakeholders, the Home Support Assessor supports coordinated service access and positive client outcomes. Through a commitment to person-centred practice, wellness and reablement principles, quality service delivery, and continuous improvement, the role plays a key part in enabling older Australians to remain safe, independent, and connected within their homes and communities.

What you will bring:

- Demonstrated experience working with older people and their families/carers,



including individuals experiencing mental illness, disability, homelessness, housing instability, or social and financial disadvantage.
- Demonstrated understanding and experience in delivering community-based aged care services using a wellness and reablement approach, including assessment, care planning, monitoring, and evaluation processes.
- Experience undertaking assessments, care coordination, and goal-orientated support planning within aged care, community, health, or human services settings.
- Demonstrated ability to exercise sound judgement, decision-making, and problem-solving skills within legislative and operational frameworks.
- Demonstrated knowledge of aged care reforms, the My Aged Care assessment pathway, and person-centred service delivery principles.
- Experience working collaboratively with service providers and stakeholders to coordinate responsive and client-focused services.
- Well-developed written, verbal, interpersonal, and stakeholder engagement skills, with the ability to communicate effectively with clients, carers, staff, and external providers.
- Strong organisational skills with the ability to manage competing priorities and work effectively within KPI and performance-based environments.
- High level computer literacy and experience working across multiple digital platforms and systems, including Microsoft Office Suite and client management systems.
- Local knowledge and/or the ability to build networks with community aged care providers and stakeholders within the region.
- Previous experience working within My Aged Care, aged care assessment services, or care coordination roles (desirable).
- Experience working within hybrid, remote,



or regionally based service delivery models (desirable).

Requirements:

- Current Queensland Class C Driver?s Licence.
- NDIS Worker Screening Check or current Yellow Card and Blue Card.
- Certificate IV, Diploma, or tertiary qualification in a relevant discipline such as Community Services, Aged Care, Disability, Allied Health Assistance, Social Sciences, Human Services, or another related field; or an equivalent combination of qualifications, skills, and demonstrated experience.
- Evidence of annual influenza vaccination in accordance with Department of Health and Aged Care requirements.
- Current First Aid and CPR Certificate.
