Position: Community Worker
Salary: $79,000 - $90,000 per annum plus 12% superannuation
Hours: 38 hours per week
Mode: Full-time
Location: Unit-405, Level-4 20 Macquarie Street, Parramatta, NSW 2150

Altura Care Pty Ltd is a trusted provider of disability support and community care services, committed to empowering individuals to live independently and participate actively in their communities. We are seeking a compassionate and experienced Community Worker to join our team and support participants in achieving their personal goals through individualised, person-centred services.
Key Responsibilities
include but not limited to:
- • Assess participants' individual needs, strengths, and support requirements to develop person-centred support plans that promote independence and community participation.
- • Provide practical support, guidance, and advocacy to participants and their families, assisting them to access appropriate disability, health, housing, employment, education, and community services.
- • Monitor participants' progress, review support outcomes, maintain accurate case notes, and recommend adjustments to support plans where required.
- • Liaise with support coordinators, allied health professionals, government agencies, community organisations, and other service providers to coordinate appropriate support and referrals.
- • Encourage and support participants to engage in community, recreational, educational, and social activities that enhance inclusion, wellbeing, and independence.
- • Assist participants in developing life skills and accessing community resources to achieve their personal goals and improve their quality of life.
- • Identify barriers affecting participants' wellbeing and implement appropriate support strategies while escalating complex issues when necessary.
- • Prepare reports, participant records, and other documentation in accordance with NDIS Practice Standards, organisational policies, and funding requirements.
- • Contribute to community engagement initiatives by promoting available services, building partnerships, and increasing awareness of community resources.
- • Ensure all services are delivered in accordance with NDIS Practice Standards, relevant legislation, workplace health and safety requirements, and Altura Care's policies and procedures.
Qualifications
- • Relevant qualification in Community Services, Social Work, Disability, Welfare, or a related discipline
- • Minimum 1 year experience in community services, disability support, case management, or a similar role
Skills and Other Requirements
- • Strong understanding of person-centred practice, community engagement, and participant advocacy.
- • Knowledge of the NDIS, community support services, and relevant Australian legislation and practice standards.
- • Excellent communication, interpersonal, and relationship-building skills.
- • Strong organisational, problem-solving, and case management abilities.
- • Ability to prepare professional reports, maintain accurate documentation, and manage confidential information.
- • Current Working with Children Check (where applicable), NDIS Worker Screening Check, First Aid and CPR certification (or willingness to obtain), and a current Australian driver's licence.
