A growing home care provider in Rockdale is seeking a compassionate and organised Care Partner to join the team during an exciting period of expansion.
With client numbers continuing to grow, this provider is recruiting two Care Partners to support its expanding Home Care Program. This is a rewarding opportunity for someone who enjoys building relationships, coordinating services and making a genuine difference in the lives of older Australians.

About the Role
As the primary point of contact for your allocated clients, you will coordinate and manage Home Care Package services while building strong relationships with clients, families and external providers.
This role combines client relationship management, care coordination, service planning and ongoing communication within a supportive and collaborative environment.
This is predominantly a phone-based role, with approximately 80% of your day spent speaking with clients, families and stakeholders.
Key Responsibilities
- Manage a portfolio of Home Care Package clients
- Build robust relationships with clients and their families
- Coordinate services to ensure individual care needs are met
- Develop and review care plans in partnership with clients
- Liaise with internal teams and external service providers
- Monitor client wellbeing and respond to changing care needs
- Maintain accurate documentation and ensure compliance requirements are met
- Advocate for clients to ensure they receive high-quality, person-centred care
About You
You are a compassionate, organised and relationship-focused professional who genuinely enjoys supporting older people to remain independent in their own homes.
You will also bring:
- Minimum 2 years' experience in a Care Partner, Care Coordinator, Case Manager or similar role within aged care, home care or NDIS
- Experience managing a client caseload
- Confidence communicating with clients and families over the phone
- Strong organisational and time management skills
- Experience working within aged care, home care, disability or community services
- A current driver's licence and your own car
