UnitingCare Queensland is seeking a Supported Living Facilitator to work in our FaDS team in Chermside West. The role involves supporting people with disability to build life skills, promoting independence and participation in daily activities.

You will collaborate with families and networks to help clients reach personal goals and maintain autonomy in everyday living. Applicants should have a current CPR and First Aid certificate and a valid NDIS Worker Screening check.
