Care Partner / Care Coordinator (Registered Nurse)

We're looking for an experienced Registered Nurse to lead our ageing-in-place program within our retirement community in Penshurst.
This is a rewarding opportunity for an RN who enjoys building relationships, coordinating care and improving the everyday lives of older Australians - without shift work.
Opportunity
Working 20-25 hours per week (4-5 days), you'll coordinate care services that help residents remain independent while ensuring they receive the right support, at the right time.
Typical hours
Monday to Friday (4-5 days)
Approximately 9.00am-2.00pm or 9.30am-2.30pm

Key Responsibilities
- Lead Vision's ageing-in-place program within the community
- Complete resident assessments and develop individualised care plans
- Coordinate care staff, service delivery and daily rostering
- Build trusted relationships with residents, families and health professionals
- Work closely with Support at Home providers and community services
- Identify changing resident needs and coordinate timely support
- Promote resident wellbeing, choice and community participation
- Maintain accurate clinical documentation and care records
- Provide hands-on personal and clinical care when required
About You
You're an experienced Registered Nurse who combines sound clinical judgement with a practical, people-focused approach.
You'll Bring
- Current AHPRA registration as a Registered Nurse
- Minimum three years' experience in aged care, community care or retirement living
- Strong assessment, care planning and coordination skills
- Experience leading staff and coordinating service delivery
- Excellent communication and relationship-building skills
- A proactive, compassionate and organised approach
- Current First Aid and CPR certification
- National Police Check (or willingness to obtain one)
- Retirement villages or retirement living
- Support at Home or Home Care Packages
- Care coordination or case management
- Dementia care and wellness programs
- Community-based aged care
