Wesley Mission is seeking a passionate Case Manager to join our Intensive Therapeutic Care – Significant Disability (ITC-SD) team, delivering trauma-informed, person-centered support to children and young people with significant disabilities.

You will lead permanency planning, develop and review case plans, and collaborate with therapeutic specialists, house managers and support workers in a fast-paced, multidisciplinary setting.
