Tharawal Aboriginal Corporation in NSW is recruiting two full time Care Finder positions to identify and engage with individuals in the care finder target population, helping them understand and access aged care and related supports.
Care finders will proactively outreach, support interaction with My Aged Care, guide through assessments, and arrange services with providers, while connecting people to community supports and information. Contract runs to 30 June 2029 with multiple locations.
