About the role
As a Coordinator – Primary Care, you will work directly with general practices across Western Sydney to support the implementation of quality improvement programs, digital health initiatives, training and resources. This is a predominantly field-based role where relationship management, communication and stakeholder engagement are at the heart of your success. You'll help practices identify opportunities for improvement, connect them with relevant support and contribute to better patient outcomes across the region.
What you'll do
- Build and maintain trusted relationships with general practices and primary care providers across Western Sydney.
- Support practices to implement quality improvement, digital health and primary care initiatives.
- Deliver training, information, resources and guidance to GPs, practice managers and practice staff.
- Promote and support the use of clinical tools and systems
- Coordinate with internal teams to ensure practices receive seamless support and access to WentWest programs.
- Record stakeholder engagement activities and maintain accurate data within WentWest systems.
- Monitor progress against KPIs and contribute to reporting, recommendations and continuous improvement activities.
- Support special projects, events and initiatives that enhance primary care delivery across the region.
About you
This coordinator-level role is ideal for someone who enjoys building strong relationships, working autonomously and making a tangible difference in healthcare.
You will bring:
- Experience working within a healthcare setting liaising with health professionals including medical professionals such as GPs and specialists, nursing and allied health providers
- Strong relationship-building and stakeholder engagement skills.
- Confidence communicating with a range of stakeholders, including GPs, practice managers, nurses and allied health professionals.
- The ability to influence, guide and support others through change and improvement initiatives.
- Strong organisational skills with the ability to manage competing priorities and work independently.
- Experience facilitating training,
presentations or one-on-one coaching and support.
- High attention to detail and commitment to accurate record keeping and data entry.
- Proficiency with Microsoft Office and CRM or data management systems.
- Relevant tertiary qualifications in health, business or social science and experience working with general practice or primary healthcare providers.

This role requires regular travel across Western Sydney, occasional evening and weekend work to attend networking events and stakeholder activities, and the use of a fully insured motor vehicle.
Successful applicants will be required to provide a current Working with Children Check, National Police Check, valid NSW Driver Licence and evidence of a fully insured motor vehicle.
