Moylan Care Group is a boutique company offering community support services for NDIS participants and Aged Care recipients throughout South West Sydney and Wollondilly areas. Established in 2017, our vision is to provide services that promote empowerment, independence, participation, compassion and dignity. Offering choice, flexibility and control is of utmost importance to us.

We are looking for a dynamic skilled to join our management team based in our Oran Park office. This role is three days per week, 9am to 5pm, with rotating weekend on-call duties.

As a Care Coordinator, you will provide exceptional customer service whilst coordinating services for our clients. You will also be responsible for mentoring a team of skilled Support Workers, as well as matching the skills of our Support Workers with theneeds, interests and requests of our clients.

Duties include but are not limited to:

Service development: Ongoing client assessment, development & review of care plans in consultation with clients, families and key stakeholders

Manage Relationships: Liaising with our internal management team and any relevant external parties, eg support coordinators

Client Engagement: Receiving referrals and promotion of the company

Service Coordination: Coordination of the provision of quality person-centred care according to the client's needs, service and care plans, funding,



and relevant policies and procedures

Roster Coordination: Proficiently coordinate rosters on a daily basis, including making shift changes

Staff Support: Promote ongoing development, training, competence and good workplace practices with support workers. Actively participate in the recruitment and orientation of support staff. Hands on support with staff and shifts when required

After Hours Support: Out of business hours on-call work is required on a rotational basis to ensure a flexible response to client needs and after-hours phone service for crisis management with both clients and staff.

Previous employment experience in community care, disability/aged care, not for profit or community services sector

Exceptional verbal and written communication skills

Ability to multitask with strong attention to detail whilst keeping calm and focused

Well-developed problem-solving skills

Excellent time management and organisation skills

Advanced computer skills in Microsoft Office and experience using information management systems

Current NSW Working with Children Clearance (or willing to obtain)

Current NDIS Worker Clearance(or willing to obtain)

Current NSW Drivers Licence and reliable motor vehicle

Tertiary qualifications in allied health, social sciences or equivalent or Certificate IV in case management or equivalent or equivalent skills and experience in disability or aged care
