Support Coordination suits a particular kind of person.
Someone who likes making sense of complicated systems, notices what has not been followed up and wants to see things through rather than only helping in the moment.
Heart Worx is looking for a Support Coordinator who can also work as a Psychosocial Recovery Coach.
Previous experience in both roles is not required. We are looking for someone with the judgement, organisation and communication skills to become very valuable at the work.

Heart Worx provides Support Coordination, Psychosocial Recovery Coaching and direct support across the Tamworth region.
We are a small, lived-experience-led organisation built around clear communication, realistic workloads and participant choice.
Our team includes people who have personally navigated the NDIS, mental health systems or caring roles. We use that understanding carefully, alongside clear boundaries, documentation and accountability.
Our work helps participants understand and use their NDIS plans, coordinate the people involved and make support arrangements more workable in everyday life.
This role centres on planning, communication, advocacy and follow-through. It is not direct care, therapy or an on-call crisis role.
The role
You will work alongside participants to:
- understand and use their NDIS plans
- coordinate providers, plan managers, clinicians and other supports
- track actions, budgets, evidence and agreed outcomes
- identify gaps or problems before support arrangements break down
- prepare for plan reassessments and changes in circumstances
- communicate clearly across the participant's support network
- maintain timely case notes, reports and records
- support participant decision-making without taking over
You will begin with structured onboarding and shadowing,
moving towards an independent caseload as your knowledge and confidence develop.
This work suits curious, organised thinkers who enjoy solving practical problems, keeping track of detail and seeing matters through.
What you will bring
You may already work in Support Coordination or Psychosocial Recovery Coaching.
You may also be coming from disability, mental health, case work, employment services, youth, housing, community services or team leadership.
You will need:
- strong writing and communication skills
- confidence using email, digital systems and detailed records
- the ability to organise competing actions and follow them through
- sound judgement and willingness to ask for another view early
- a grounded understanding of disability, mental health or community work
- curiosity and willingness to learn complex NDIS systems
- the ability to maintain professional boundaries without becoming cold or procedural
- relevant paid or substantial volunteer experience
- a current NDIS Worker Screening Check, Working with Children Check and driver's licence, or the ability to obtain them
- access to a reliable and appropriately insured vehicle
Direct Support Coordination or Recovery Coaching experience is welcomed, but it is not the only pathway into the role.
Lived experience is respected within Heart Worx. Nobody is expected to disclose personal information or use their story to prove they are suitable.
