BlueCare in Yeppoon is seeking a Care Partner to ensure person-centred, high-quality community care. You will oversee care planning and coordination, working with clinical teams to tailor services to each client's funding and needs.

The role involves developing and reviewing care plans, managing budgets and coordinating multidisciplinary activities, liaising with clinicians, and assisting clients to navigate available resources. Most days are on the road delivering home- and community-based care.
