We are looking for Homecare RN to join our team for Homecare SUPPORT

Core Position Description (JD)

Clinical Assessments: Conduct initial and ongoing health assessments, fall-risk evaluations, and functional capacity reviews.

Care Plan Management: Develop, implement, and adjust individualized client care plans in collaboration with case managers and families.

Clinical Nursing: Perform complex nursing tasks such as wound management, catheter care, palliative support, and medication administration.

Team Supervision: Guide and oversee the care delivered by Enrolled Nurses (ENs) and Assistants in Nursing (AINs).

Client Advocacy: Educate clients and families on chronic disease management and act as a liaison with general practitioners and external healthcare services.
