Nepean Blue Mountains Local Health District (NBMLHD) seeks a compassionate Clinical Aged Care Assessor to work within a multidisciplinary team. You will assess care needs of older people, develop person-centred recommendations, and support access to aged care services that promote independence and wellbeing.

Engage with families, carers and healthcare providers to ensure timely, appropriate care across hospital and community settings.
