About the Role: We are seeking an experienced and motivated Support Coordinator to join our team. This is an excellent opportunity for someone with existing experience who is looking to further develop their skills within a supportive and collaborative setting. Key Responsibilities:
Manage a caseload of participants, supporting them to understand and implement their NDIS plans
Coordinate services and supports tailored to individual participant needs
Build and maintain positive relationships with participants, families, and service providers
Promote participant independence, choice, and empowerment
Maintain accurate case notes, records, and individualised plans
Prepare reports and documentation to support NDIS plan reviews
Collaborate with internal teams, external professionals, and community organisations
Advocate for participants to ensure their goals, preferences, and needs are met
Work closely with senior Support Coordinators to ensure quality service delivery
About You:
Demonstrated experience as a Support Coordinator (NDIS environment preferred)
Robust interpersonal, communication and organisational skills
Someone enthusiastic and eager to learn OR someone with Disability experience
A person-centred and empathetic approach to support delivery
Ability to work collaboratively within a team and follow direction
A proactive attitude and willingness to learn and grow
Mandatory requirements:
NDIS Worker Screening Check
Working With Children Check
National Police Check
Current First Aid Certificate
Valid driver’s licence and access to a reliable vehicle
Qualifications:
Minimum Certificate IV in Community Services, Social Services, Mental Health, Allied Health, or a related field
