Sydney Region Aboriginal Corporation (SRAC) is a community-controlled, not-for-profit Indigenous organisation dedicated to improving the health, social, emotional, and economic wellbeing of Aboriginal people. Established in 1996, SRAC operates a Community Hub in Penrith and a Child & Family Centre in Doonside, offering a range of culturally safe and person-centred services to address disparities in education, health, and wellbeing for Indigenous communities.
We are seeking a highly motivated Elder Care Support Coordinator to support identified clients to navigate access to appropriate local aged care support services.
The Elder Care Support Coordinator will provide advocacy support to ensure clients receive a culturally safe plan assessment, support clients to understand their aged care service entitlements, including My Aged Care registration, contractual arrangements, and costs, and help ensure Elders and their family are well-prepared for plan reviews and outcomes.
Underpinning the role are two key work practices: cultural safety and trauma informed practice.
The Elder Care Support Coordinator will have an understanding of the historical and systemic issues affecting Aboriginal and Torres Strait Islander people and communities, and build genuine trust through purposeful yarning, respecting Aboriginal community knowledge and protocols, and ensuring Elders retain self-determination over their care pathways.
This position will be based in our SRAC Hub located in Penrith.
Selection Criteria:
- Aboriginality. This position is an identified Aboriginal position. Aboriginality is a genuine qualification authorised under Section 14(d) of the Anti-Discrimination ACT 1977
- An understanding of the issues experienced by older Aboriginal people living in the community
- Certificate in Community Services or Aged Care or previous relevant experience
- High level of verbal and written communication skills, experience in recording case notes and use of client management database
- Demonstrated skill in working with people with complex needs, and can maintain skilled boundaries
- Familiar with local Aboriginal communitiesand community service providers in Nepean/Blue Mountains and Greater Western Sydney
- Must have or be willing to obtain current Police and Working with Children Checks
- Musthold a NSW unrestricted driver licence
