We are BCR Communities – a not-for-profit award winning community organisation serving communities within the Illawarra and Shoalhaven for 35 years.
Due to sustained growth in demand for our services we are looking for an experienced and qualified Care Partner to join our team and deliver person centered case management support to participants receiving home care services through the Support At Home program. This role is full time, Monday through Friday, based predominately at South Nowra with the option of a hybrid work from home arrangement. The role involves visits to participants in their homes and in the community throughout our service region.
You will be supporting participants to develop healthy ageing and wellness goals in their care plans, in accordance with the Support at Home Program, Aged Care Act and Strengthened Aged Care Quality Standards. You will achieve this through working in a multidisciplinary team, forming strong professional relationships with participants and their support network, whilst working closely with all suppliers and health professionals involved in their care.
We are specifically seeking someone who will bring a track record of supporting individuals with diverse and complex care needs and who hold a minimum of a Certificate IV in Ageing Support (or equivalent).
To be successful in this role you will be someone who:
- Collaborates with participants, listening with empathy to their needs, goals and preferences, and supporting them to exercise choice
- Exhibits excellent assessment, documentation, and care plan implementation skills, including budgeting
- Communicates effectively with exceptional negotiation and interpersonal skills
- Thrives in a fast-paced environment with excellent prioritisation, time management and organisational abilities
- Is a proactive problem-solver with a commitment to maintaining confidentiality
- Displays a passion for delivery of a restorative wellness model, which aims at improving the quality of life for older Australians
- Is seeking and committed to a positive team and workplace culture reflecting our vision, mission and values
- Demonstrates business acumen and computer skills, including MS Word, Excel, Outlook, Teams and client management systems
- Has experience in the care sector and holds a tertiary qualification in aged care, health and/or allied health (remuneration will be commensurate with experience and qualification)
- Holds a First Aid certificate
