Your Role

As a Clinical Care Coordinator,



you'll take a hands-on leadership role in clinical care delivery while supporting and mentoring the nursing and support teams.

- Manage care planning and assessments
- Lead a team of RN's, EN's and Support staff
- Managing complex wound care and coordinating allied health referrals
- Oversee quality and carry out education for clinical staff.
- Partner with families and allied health teams to deliver holistic care
- Foster a safe, supportive, and resident-focused workplace culture

About You

- Bachelor of Nursing with current AHPRA registration
- 1-2 years experience working as a Senior Nurse/Clinical Coordinator
- Proven ability to provide strong clinical leadership, mentoring, and support to nursing and care teams
- Sound knowledge of accreditation standards, compliance requirements, and care planning processes
- A genuine commitment to delivering high-quality, resident-centred care
