Aged Care Service Coordinator – Support at Home

About the role
We're looking for an experienced Aged Care Service Coordinator to manage a portfolio of Support at Home customers, delivering high-quality care management and service coordination that keeps people secure, independent, and living life on their terms.
You’ll work in partnership with customers, families, support workers, allied health professionals, and external providers to ensure services are person-centred, outcome-focused, and compliant with the Aged Care Act 2024, the Strengthened Aged Care Quality Standards, and the Support at Home Program Manual.
What you’ll be doing
- Managing an allocated portfolio of Support at Home customers, from intake assessments through to ongoing care planning and reviews
- Developing person-centred care plans that reflect customer goals, preferences, and assessed needs
- Monitoring customer budgets and funding, identifying risks before they become problems
- Coordinating service delivery across support workers, allied health, and external providers
- Completing assessments and referrals, including Home Safety and Falls Risk Assessments
- Maintaining accurate, timely documentation that meets legislative and audit requirements
- Supporting continuity of care, including hospital admissions, discharge planning, and respite coordination
- Contributing to a culture of continuous improvement, safety, and inclusion
What you’ll bring
- Certificate IV in Ageing Support, Community Services, or equivalent
- Minimum 2 years' experience in aged care, with demonstrated care management experience
- Working knowledge of the Support at Home Program and the Aged Care Act 2024
- Current Australian Driver Licence, National Police Check, First Aid/CPR, and relevant screening checks
- Strong assessment, care planning, and budget monitoring skills
- Excellent communication and documentation skills, with the ability to manage competing priorities
- A Diploma or Degree in Nursing, Social Work, Allied Health, or Human Services
- Experience using My Aged Care and Dynamics 365 (or similar client management systems)
