Empowering You Disability Services (EYDS) is a registered NDIS provider committed to delivering high-quality, person-centred supports that empower people to live their best lives. As our organisation continues to grow, we are looking for an experienced, motivated and compassionate Case Manager to join our dedicated team.
This is an exciting opportunity for someone who enjoys building meaningful relationships, coordinating supports, and ensuring participants receive the highest standard of service.
About the Role
As a Case Manager, you will be responsible for managing a caseload of NDIS participants across a range of supports. You will work closely with participants, families, guardians, support workers and allied health professionals to ensure services are coordinated effectively and participant goals are achieved.
You will play a key role in maintaining participant wellbeing, ensuring compliance with NDIS Practice Standards, and supporting positive outcomes through proactive case management.
Key Responsibilities
- Manage an allocated caseload of NDIS participants.
- Develop and maintain positive relationships with participants, families and stakeholders.
- Coordinate participant supports and services to meet individual goals.
- Complete participant reviews, assessments and service planning.
- Monitor participant wellbeing and respond proactively to changing support needs.
- Develop and update care plans, risk assessments and participant documentation.
- Liaise with Support Workers to ensure supports are delivered safely and consistently.
- Work collaboratively with Rostering, HR and Operations teams.
- Maintain accurate and timely case notes and documentation.
- Participate in case conferences,
multidisciplinary meetings and incident reviews.
- Ensure compliance with NDIS legislation, Practice Standards and organisational policies.
- Identify opportunities to improve participant outcomes and service delivery.
About You
You are organised, compassionate and thrive in a rapid-paced environment. You enjoy problem-solving, building relationships and advocating for participants while balancing operational requirements.
You will have:
- Experience in Disability, Community Services, Mental Health or Aged Care.
- Previous experience in case management or a similar coordination role.
- Excellent communication and interpersonal skills.
- Strong organisational and time management abilities.
- The ability to prioritise competing demands and work autonomously.
- High attention to detail and strong documentation skills.
- Confidence using Microsoft Office and client management systems.
- A commitment to person-centred practice and continuous improvement.
Essential Requirements
- Certificate IV in Disability, Community Services, Individual Support or a related qualification (Diploma or higher highly regarded).
- Current NDIS Worker Screening Clearance.
- Current Working with Children Check (Blue Card).
- Current First Aid & CPR Certificate.
- Current Australian Driver Licence.
- Comprehensive Vehicle Insurance.
- Full Australian working rights.
