An exciting prospect has arisen for an experienced Physiotherapist to join our close-knit team on a full-time basis. You will play a vital role in helping some of the most vulnerable people in our community remain independent and live safely at home.

Multicultural Care is a not-for-profit organisation that specialises in providing home care services to frail-aged individuals, people with disabilities, and their carers from culturally and linguistically diverse (CALD) backgrounds. We are seeking an experienced Physiotherapist to work primarily with our clients across Sydney's Inner West, South East, and South West regions. These clients receive services through Home Care Packages, Commonwealth Home Support, and Short-Term Restorative Care services.

You will be responsible for conducting assessments and providing physiotherapy services in clients' homes, with a focus on assisting them to maintain or improve their mobility, strength, and overall quality of life.

Reporting to the Team Leader, Grant Funded Services, your role will involve:

- Maintaining caseloads of clients in line with Multicultural Care’s and funding requirements.
- Prescribing equipment, offering recommendations, and providing ongoing therapy support.
- Reporting outcomes and recommendations (written and verbal) in a timely manner.




- Promoting health and wellness solutions to support optimal mental and physical outcomes.
- Liaising with clients, families, referrers, doctors, and other health professionals to achieve coordinated outcomes that meet client goals.
- Managing client case ratios effectively and ensuring caseloads are met.
- Monitoring client hours of service against agreed Participant Service Funding Agreements (e.g., NDIS, HCP).
- Contributing to the service’s effectiveness by ensuring accurate and appropriate documentation is maintained, including databases such as Alaya Care.
- Ensuring compliance with MC policy and procedures in maintaining accurate medical and statistical records.
- Demonstrating strong computer literacy skills (Word, Excel, Outlook, MS Teams, and CRM).

Skills, Experience, and Competencies:

- A minimum Bachelor’s degree in Physiotherapy
- Full AHPRA registration
- A minimum of two years of clinical experience, preferably in aged care/disability services
- Demonstrated ability to work effectively with multidisciplinary teams
- Ability to assess client needs and set independence-focused goals to develop treatment plans
- A commitment to delivering high-quality, person-centred care in line with Multicultural Care’s values
- Strong communication, time management, and organisational skills
- A willingness to work independently and collaboratively within a team
- Access to own vehicle and comprehensive car insurance
- Current First Aid Certificate or a willingness to obtain
- Experience with Commonwealth Home Support and Packaged Care Programs
- Ability to speak a second language
