We are seeking a reliable and compassionate Support Workers to provide high-quality, person-centered support to NDIS and Aged Care clients in the Port Macquarie area. We are seeking a support worker who will build robust rapportand bring consistency. You’ll provide disability and social support; helping them live with dignity, independence, and joy.

Please note: These preferences are based on the participant’s individual needs, in line with the comfort, support requirements and the participant’s right to Choice and Controlunder the NDIS.

Main responsibilities and activities may include:

Provide a high quality and timely care support to the client as directed by Home Caring, and in accordance with his care plan.

Safely transport the client using your own Vehicle (own vehicle essential)

Support client in getting out for social engagement, activities and in attending appointments.

Provide personal care and daily living (domestic) assistance.

Assistance with household tasks (may include cleaning, meal preparation.)





Document service activities that support service outcomes.

Support participant in achieving individual NDIS goals.

Maintain accurate documentation (including reporting) and shift notes.

Requirements:

Professional experience in the NDIS and/or Aged Care sectors. Min 1 year experience and Cert 3 as a minimum requirement

Excellent communication skills, commitment to a person-centred focus and flexibility to support needs based on individual circumstances

Demonstrated experience providing compassionate, person-centred support to individuals living with dementia.

Fluent in English and Vietnamese (spoken) Multiple languages but fluent in English

NDIS Worker Screening Check

Working with children Check

Current First Aid certificate and current CPR certificate

Current Drivers Licence & access to a reliable vehicle- full compressive insurance
