Yahweh Care Pty Ltd is seeking a compassionate Community Worker to support people living with disability to achieve independence, wellbeing and community participation. This full time permanent role offers AU$75,000–AU$85,000 per year plus superannuation and a supportive team environment.
You will assess clients, develop tailored support plans, facilitate access to services, promote social participation and maintain accurate records in line with NDIS requirements.
