ABOUT THE ROLE
- To coordinate (as appropriate to the program) the delivery of services and supports to deliver quality person centred outcomes whilst ensuring service standards are met and programs are operated within program guidelines and key performance indicators (including financial and budget targets).
- To provide a comprehensive and holistic approach to service coordination, monitoring and leadership of direct reports and participate as a member of a team.
- Report, communicate and provide regular information to the Senior Coordinator/Service Manager in relation to the outcomes of the program/service.
- Facilitate outcomes for, clients and their families by providing a quality person-centred service, delivered with respect and integrity, with a high standard of client service.
- To continuously improve the organisation’s service delivery while taking a leading role in bringing about desirable cultural change.
ABOUT YOU
Qualifications and Credentials:
- Cert IV in relevant industry qual and/or significant industry experience.
- Current Working with Children Check (NSW)
- NDIS Worker Screening
- Satisfactory Criminal Record Check.
Experience & Knowledge:
- Demonstrated knowledge and experience in comprehensive assessment, planning, provision and evaluation of services for clients. Demonstrated expertise in the provision of services to people with Complex Support Needs eg, Challenging behaviours (behaviours of concern), High intensity medical needs
- Significant experience working in a community and/or disability setting/service (or similar).
- Knowledge and experience of working within marginalised communities with complex mental health, trauma, crisis and physical needs.
- Understanding of social disadvantage and commitment to the rights of Indigenous and other people living with disability and/or mental illness.
