For more than 14 years, we've been supporting older Australians and their families through some of life's most significant life transitions. Originally established as Aged Care Placement Services in 2012, we built our reputation by helping families navigate residential aged care with confidence, compassion and clarity.

As the needs of older Australians evolved, so did we. Today, as Elder Folks, we provide person-centred support across aged care navigation, housing support, downsizing, hoarding and squalor intervention, homelessness prevention and wellbeing services. Although our services have grown, our purpose remains the same—to ensure every older Australian can age with dignity, safety and choice.

We're looking for a compassionate and experienced professional with a minimum of three years' experience in the aged care sector to join our Programs & Service Delivery team. You'll have a strong understanding of My Aged Care, CHSP, the Support at Home program, residential aged care eligibility and admissions, and be passionate about supporting older Australians to navigate complex aged care, housing and community systems. The Client Support Coordinator provides holistic, person-centred case coordination and practical support to older Australians experiencing complex aged care, health, housing and social needs.

Working across a range of service streams, you will support clients to navigate the aged care industry, register with and access My Aged Care, coordinate services, advocate on their behalf and develop tailored support plans. Your work will help older Australians remain living safely at home, secure appropriate housing,



access the services they need to improve their wellbeing or transition into residential aged care when the time is right.

We're looking for someone with demonstrated experience working in the aged care sector and supporting older people experiencing complex or vulnerable circumstances. You will be an empathetic communicator, a strong advocate and an organised coordinator who can build trusted relationships with clients, families, colleagues, healthcare professionals, government agencies, community organisations and aged care providers.

About the Role

As a Client Support Coordinator, no two days are the same. One day you may be helping an older person access My Aged Care and organise in-home supports. The next, you could be supporting someone experiencing housing insecurity, coordinating a relocation or helping a family navigate the transition of a loved one into residential aged care.

You’ll manage a diverse caseload, undertake client assessments, develop person-centred support plans, coordinate services across multiple providers and advocate to achieve the best possible outcomes for every client.

Key responsibilities include:

- Providing aged care navigation and case coordination for older Australians with complex needs.
- Supporting clients to access medical and allied health services, Support at Home, Commonwealth Home Support Programme (CHSP) services and residential aged care.
- Coordinating health, housing, community and in-home support services.
- Advocating for clients to achieve timely access to services and appropriate supports.
- Supporting clients through housing transitions, downsizing, relocation or moving into retirement villages.
- Building and maintaining strong relationships with hospitals, aged care providers, housing organisations, government agencies and community partners.
- Maintaining accurate documentation, case notes and client records while ensuring compliance with organisational and legislative requirements.
