One Dream Community is seeking an experienced and compassionate Registered Nurse or Enrolled Nurse to join our Queensland team in Springwood as an Aged Care Coordinator.

This role primarily involves coordinating care and services for older people receiving support through the Support at Home program. You will conduct client assessments, develop and review care plans, coordinate services, monitor changes in clients’ needs and work closely with clients, families, care workers and other health professionals.

The position combines clinical knowledge, care coordination and administrative responsibilities. It is ideal for an RN or EN with community aged care experience who is confident working independently and supporting culturally diverse clients in their own homes.

About the role

You will conduct client assessments, develop and review care plans, coordinate services, monitor changes in clients’ needs and work closely with clients, families, care workers and other health professionals.

Key responsibilities

- Conduct home visits and comprehensive assessments of clients’ health, functional ability, risks and support needs.

- Develop, implement, monitor and review individualised care plans.

- Coordinate Support at Home services in accordance with clients’ assessed needs, preferences, budgets and service agreements.

- Regularly monitor clients’ wellbeing, progress and changing care needs.





- Identify clinical deterioration, risks or gaps in care and elevate concerns appropriately.

- Coordinate referrals and services with general practitioners, allied health professionals, hospitals, pharmacies and other service providers.

- Support clients and families to understand available services and make informed care decisions.

- Ensure services are culturally appropriate and person-centred, including communicating with Korean-speaking clients and families where applicable.

- Work closely with scheduling, coordination, finance and service delivery teams to ensure continuity of care.

- Maintain accurate and up-to-date assessments, care plans, progress notes, risk assessments and other client records.

- Complete incident reports and follow-up actions within required timeframes.

- Participate in audits, case conferences, quality improvement activities and regulatory compliance processes.

- Support the onboarding, training and competency assessment of care workers.

- Provide clinical guidance to care workers and monitor whether services are being delivered safely and in accordance with care plans.

- Promote best practice in aged care, clinical governance, infection prevention, medication support and client safety.

Clinical responsibilities

- Medication assessments and medication management support.

- Wound assessment and wound care.

- Catheter care, including SPC and IDC support.

- Continence assessment and management.

- Clinical observations and health monitoring.

- Development of clinical support plans and escalation pathways.





- Training and competency assessment of care workers undertaking delegated or clinical support tasks.

What we’re looking for

- Current registration as a Registered Nurse or Enrolled Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).

- Demonstrated experience in community aged care, home care or a similar care coordination role.

- Sound understanding of person-centred care and the Support at Home environment.

- Strong assessment, care planning, coordination and clinical documentation skills.

- Ability to identify and respond appropriately to clinical risks and changes in a client’s condition.

- Excellent communication, organisational and interpersonal skills.

- Ability to work independently while collaborating effectively with a multidisciplinary team.

- Compassionate, respectful and culturally responsive approach to supporting older people and people with disability.

- Confidence using electronic client management and documentation systems.

- Current driver’s licence and ability to travel to clients’ homes.

- Bilingual English and Korean communication skills are highly regarded.

- Experience in medication management, wound care, SPC and IDC care is desirable.
