Children's Health Queensland - Logan Central QLD

The Healthy Kids Club service provides care to children and young people who are living with overweight, obesity, and/or Type 2 Diabetes Mellitus. The community-based clinic services the multicultural Logan population to improve the health outcomes of these families.

The multidisciplinary team of the Healthy Kids Club provides culturally responsive, family centred care via consultations, home visits, school visits, telehealth, and group sessions. The team aims to empower patients improve their health and achieve their goals through education, skill development, and holistic behaviour change. The population is complex requiring careful consideration when planning interventions, as well as maintaining continuum of care across other services.

About the Role

In this role you will deliver quality outcomes for children, young people and families (Leader of Self) in accordance with the Children's Health Queensland Leadership Excellence Framework and the Queensland Public Service and the CHQ values. Your contribution will include:

- Deliver quality clinical services within an overweight, obesity and type 2 diabetes mellitus caseload in accordance with prescribed professional and ethical standards.
- In a community health setting, support children, young people, and families to implement healthy lifestyle changes.
- Provide family-centred, individualised therapy as well as facilitate group interventions within the Healthy Kids Club service.
- As experience builds, apply increasingly independent clinical judgement to more complex clinical decision making, under the clinical practice supervision or professional guidance of a more senior clinician.
- Work cooperatively and effectively within the multidisciplinary team and with patients.
