Thriveon Support Services is a person-centred NDIS provider based in South East Queensland. We support participants with daily living, community access, personal care, domestic assistance and transport - and we do it properly, with genuine care for the people we work with.

The Role

We are looking for a dedicated and reliable Support Worker to join our growing team. You will be working one-on-one with NDIS participants in their homes and in the community, helping them achieve their goals and live as independently as possible.

No two days are the same. You might be supporting someone with their morning routine one day and accompanying them to a community activity the next. What stays the same is the standard of care we expect, respectful, consistent and person-centred, every time.

Key responsibilities:

- Providing support with daily living tasks including personal care, meal preparation and household tasks
- Supporting participants to access the community, attend appointments and engage in social activities
- Transporting participants to and from activities as required




- Maintaining accurate progress notes after each shift
- Communicating openly with the team and raising any concerns promptly
- Adhering to Thriveon's policies, procedures and the NDIS Code of Conduct at all times

What We Are Looking For

- Current NDIS Worker Screening Clearance (or willingness to obtain prior to starting)
- Valid Queensland driver's licence and reliable vehicle with business use insurance
- First Aid and CPR certificates (or willingness to obtain)
- A genuine commitment to person-centred, dignified support
- Strong communication skills and a qualified attitude
- Ability to work independently and manage your time effectively
- Experience in disability support, aged care or a similar field is preferred but not essential, we will train the right person
