Duties

- Effectively engage clients (extended family and kin of child) through cold-call home-based visits. Turn initial ‘kin engagement’ home visit into ongoing consent and interest in becoming a Kin Carer.
- While completing family engagement activities, ask to identify other solid family members that could possibly become a kin carer. Actively seek contact details and relationship connection with child and broader family.




- Engage the child and/or lead Residential Care Worker in determining the views and wishes of family placement options with family, as directed by line Manager. Utilise the Narrative Assessment Tools to assess barriers and wishes.
- Assist in the assessment of carers, including initial screening, Blue Card submission, and PAC assessments. Advocate and support families when being assessed externally.
- Develop a comprehensive Transition Plan (Case Plan) that explicitly addresses all concerns outlined in the (1) referral, (2) program assessment tool, (3) Transition NAT tools, (4) Child Assessment, and (5) Carer Assessment.
- Deliver case managed services according to the program model of care, ensuring minimum case load requirements are maintained to ensure equitable access to services for community.
- Maintain accurate and detailed case notes of all client interactions, ensuring case notes are written from the perspective of clients in their voice.
- Ensure clients complete the ‘Have Your Say’ client feedback tool and submit. Act on the client feedback to make continuous improvements to your practice and approach with clients.
- As per MOBnet Performance module, participate in monthly prescheduled supervision with direct line Manager. Ensure Supervision tasks are the priority in work completion. Ensure all associated monthly supervision tasks in MOBnet are completed (two-way feedback, team culture score etc).

Culture

- Base Salary of $105,673 per annum (Based on qualification and experience).
- 12% Super + access to salary packaging (Increases your take home pay).
