Aged Care Manager / Aged Care Partner (Full Time role in Office in Narrabeen)

Daughterly Care Community Services is a leading Aged Care Provider of In-Home Care that enables older people to stay at home for life and avoid the Nursing Home. This year, we celebrate 27 years in the Aged Care sector.

Join our success story! Our business is expanding and we are looking for an experienced and motivated Aged Care Manager to liaise with our clients and our Caregivers, coordinating the provision of in-home care to enable clients to live safely, comfortably and joyfully at home. You will have the ability to develop relationships with our elderly clients from the very first phone call and really make a difference in their lives.

The successful applicant will have experience in a similar role working in a challenging environment with the ability to hit the ground running because this role will be interesting, rapid-paced, and requires a high-level of organisation. We are specifically seeking applicants with clinical, social work, or any comparable care-coordination experience.

As an Aged Care Manager / Aged Care Partner, your key responsibilities of this role are:

- Assess, identify and resolve practical care solutions for clients and caregivers including managing a clients total care needs, rostering (minimal) and WH&S;
- Document, monitor and review client goals, preferences and needs demonstrating superior time management skills and maintain up to date case notes;
- Develop,



implement and review care plans in collaboration with clients and their representatives with the ability to strengthen and maintain relationships;
- Monitor the delivery of services to ensure they are aligned with Aged Care Standards, policies and procedures and promote dignity and respect;
- Advocate for our elderly clients and their representatives and assist them to make informed decisions;
- Maintain a positive, person-centered approach with a genuine passion for improving the day-to-day wellbeing of our elderly clients;
- Growing and maintaining relationships to provide excellent in-home care to our clients;
- Experience with managing Support at Home Packages (SaH) and budgets is advantageous;
- Communicating with Caregivers and clients via a high number of emails/phone calls; and
- Liaise, strengthen and develop relationships with clients and caregivers.

To be considered for this role, it is essential you have:

- Minimum 2 years’ experience working in an office environment with an Aged Care provider;
- Competent IT skills e.g. Outlook and Word (we will provide full training in our own database);
- A kind heart and genuine passion for providing high quality care to the elderly;
- Strong customer service skills, friendly telephone manner and highly organised;




- Superior time management and attention to detail;
- The ability to change direction and be flexible due to priority changes;
- Excellent verbal and written communication skills;
- Excellent problem-solving abilities and critical thinking skills;
- Ability to multi-task with a positive, happy and can-do attitude; and
- Vaccinated with Covid-19 vaccine (minimum 2 vaccines).
