About the role

Communify is seeking an experienced and capable In Home Support Coordinator to lead the day-to-day delivery of our Commonwealth Home Support Programme (CHSP).

This is a key operational leadership position responsible for coordinating safe, responsive and person-centred services that support older people to remain living independently in their homes and communities.

Reporting to the Aged Care Senior Manager, you will lead and support a team of Support Workers and administrative staff while overseeing service coordination, workforce planning, client outcomes, quality, compliance and continuous improvement.

Our In Home Support Program provides practical services—including home maintenance, home modifications, home security solutions, information, advice and referrals—to help older people live safely, independently and confidently at home.

Your key responsibilities

In this varied leadership role, you will:

- Provide day-to-day leadership, supervision, coaching and support to Support Workers and administrative staff.
- Foster an engaged, accountable and high-performing team culture.
- Oversee service coordination, rostering, workforce capacity and allocation of clients to appropriately skilled workers.
- Coordinate client intake, assessments, support planning and regular service reviews.
- Monitor client outcomes, service quality and customer satisfaction.
- Build positive relationships with clients, families, carers, contractors, health professionals and community partners.
- Act as an escalation point for client feedback, concerns and complaints.




- Maintain accurate client, workforce and service information within AlayaCare and other organisational systems.
- Monitor operational performance, funding requirements, service capacity and resource utilisation.
- Ensure compliance with the Aged Care Act 2024, Strengthened Aged Care Quality Standards and CHSP requirements.
- Support incident management, audits, accreditation, safeguarding and continuous improvement activities.
- Participate in an on-call roster outside standard business hours when required.

About you

You are an experienced community aged care professional who combines strong operational coordination skills with genuine, people-focused leadership. You are organised, calm under pressure and able to balance client needs, workforce demands, compliance requirements and service priorities.

You will bring:

- A Certificate IV in Ageing Support, Individual Support, Community Services or a related discipline, or an equivalent combination of qualifications and demonstrated experience.
- Demonstrated experience leading and supervising staff within aged care, community services or a health environment.
- At least two years’ experience delivering or coordinating services through CHSP and/or the Home Care Packages Program.
- At least two years’ experience in service coordination, workforce management, client assessment, care planning and service review in community aged care.
- Sound knowledge of the Aged Care Act 2024,



Strengthened Aged Care Quality Standards, CHSP and My Aged Care.
- Knowledge of medication management requirements in community aged care.
- Strong communication, relationship-building, decision-making and problem-solving skills.
- High-level organisation and time-management skills, with the ability to manage competing operational demands.
- Confidence using Microsoft Office and client management systems such as AlayaCare.

A diploma in community services, leadership and management, case management, aged care coordination, health services management or a related discipline will be highly regarded.

Mandatory requirements

The successful applicant must hold or be willing and eligible to obtain:

- A current Queensland Driver Licence.
- A current First Aid and CPR Certificate.
- A Blue Card and NDIS Worker Screening Clearance.
- A satisfactory National Criminal History Check.

You must also be able and willing to travel throughout the service area and participate in an on-call roster when required.
