About the Role We are seeking an experienced, motivated Practice Nurse to join a welcoming, family-oriented general practice in Molong. This is a hands-on, autonomous role for a registered nurse who enjoys a varied clinical workload, an innovative environment, and a collaborative team committed to patient-centred primary care.

Key Responsibilities

Provide nursing support for consultations, chronic disease management, wound care, immunisations, and minor procedures. Perform clinical assessments, triage, and point-of-care testing in line with practice protocols.

Deliver health promotion, education, and preventive care tailored to diverse community needs. Support care planning and review for patients with diabetes, COPD, cardiovascular disease, and mental health conditions. Undertake recall and reminder systems, chronic disease registers, and quality improvement initiatives.

Collaborate with GPs, allied health professionals, and administrative staff to optimise patient flow and outcomes.





Who We're Looking

For Registered Nurse with current AHPRA registration and relevant primary care experience. Excellent communication skills, empathy, and a patient-first approach. Comfortable working autonomously and contributing to a high-performing, friendly team.
