About the Client Our client is seeking a compassionate Community Worker to deliver holistic support that empowers clients to achieve independence and wellbeing.
You will work within a collaborative team, assessing needs, coordinating services, and advocating for vulnerable individuals across the community.
Key Duties and Responsibilities
- Conduct comprehensive client assessments to identify physical, emotional, social, and financial needs; then develop, implement, and review individualized support and education plans.
- Interview clients and their families to understand the nature and extent of difficulties, ensuring culturally sensitive and client-led approaches.
- Monitor and document client progress, adjusting interventions as required, and report outcomes to relevant stakeholders.
- Refer clients to appropriate agencies for additional help – including housing, healthcare, employment, and disability services – and follow up to ensure continuity of care.
- Assess community needs and resource gaps in health, welfare, housing, and training, and liaise with community groups, government bodies, and private businesses to promote awareness of available services.
- Provide direct support, education, and advocacy for families, children, and disabled persons in home, group housing, or community settings.
- Assist young people with social, emotional, and financial challenges through counselling, life skills training, and referrals.
- Prepare funding submissions, grant applications, and detailed reports for government bodies and other agencies.
Requirements
- Diploma or higher in community services, social work, or a related field (or equivalent experience).
- Minimum one-year relevant experience in community services, social work, or disability support.
- Strong verbal and written communication skills, with the ability to build trust with diverse clients and stakeholders.
- Knowledge of the NDIS, aged care, and mental health sectors is highly regarded.
- Current driver’s license and valid working with children check.
