Social Worker – Huntington’s Disease Community Practitioner

We deliver a range of services including NDIS supports, individual and family support, community programs, education and training, and advocacy across Australia.

As part of our Community Programs team, this is a rewarding and diverse role supporting individuals and families at every stage of the HD journey. You will provide practical assistance, emotional support, and capacity‑building interventions, while working collaboratively with health, disability, and community services to enhance quality of life.

About You

You are passionate about making a meaningful difference and thrive working as part of a multidisciplinary team.

You bring experience in health, disability, or community services and are confident delivering high‑quality, person‑centred support across NDIS, fee‑for‑service, and community program settings. This may include non‑clinical counselling, as well as delivering training, education, or information sessions to a range of audiences.

A minimum of 2 years of professional experience in health, disability, or community services is required to effectively deliver high‑quality support across NDIS, fee‑for‑service, and community program settings.

You are organised, collaborative, and comfortable using CRM systems to support effective service delivery. Experience working with people living with disabilities, neurological or degenerative conditions, or with complex care needs, will be highly regarded. NDIS experience is strongly desirable.

- Current registration with the Australian Association of Social Workers (AASW)
- Working with Children Check
- NDIS Worker Screening Check

Key Responsibilities

- Provide social work support, non‑clinical counselling, outreach, and advocacy
- Deliver information, resources, and programs to support individuals and families
- Facilitate education and training for professionals, providers, and carers
- Lead support groups that foster connection and shared learning
- Contribute to community awareness initiatives and education
- Maintain accurate case notes, reporting, and billing in line with NDIS and organisational requirements

Applicants must be Australian citizens or permanent residents and hold current WWCC and NDIS Worker Screening clearance, qualification in Social Work and have current registration with the Australian Association of Social Workers (AASW).
