Care Partner - Clinical and Complex Care (Care Manager)

ABOUT THE ROLE

As Care Partner – SIL & Complex Care, you will hold clinical governance for the residents of our specialist dementia and complex medical need residential home (permanent and short-term/respite stays), alongside a caseload of complex community clients with high intensity, RN-managed support needs that are living in their own homes.

You are the clinician who turns an assessment into a plan, a plan into a properly funded service agreement, and that agreement into safe, well-supervised support on the ground. You will work alongside a Workforce Planner, so the rostering and award side is handled and your sole focus is the client and clinical side of coordination, and ensuring the workers on the ground are adequately trained.

WHAT YOU'LL DO

- Be the trusted point of contact, make-it-happen person, and the one with the answers and solutions for our clients navigating complex care and their families
- Care planning,



clinical assessment and risk assessment for clients living in our residential care facilities and clients living in their own homes with complex care needs
- Design care programs to suit the needs of consumers within budget and funding allowances and manage budgets and planning
- Develop strong, real-life care plans, clinical directives and high-intensity support protocols that a support worker can actually follow at 3am
- Complete intake and admission process for consumers including assessments, home visits, care planning and service design, agreeements and funding
- Deliver clinical education and training to the support workers on your clients and supervise and oversee high intensity support requirements
- Manage support staff on clinical and service delivery matters including support and supervision, performance reviews, supervision and performance management
- Conduct scheduled and unscheduled care plan reviews and reassess whenever needs, funding or risk change
- Grow your client portfolio by building and maintaining relationships and sharing the JJ Story with clients, representatives and industry partners
- Participate in our rotating on call roster, being on call to support our workers over the phone outside business hours one weekend and five weekdays a month.

WHAT YOU'LL BRING

- Current AHPRA registration as a Registered Nurse




- Experience in complex or high intensity clinical care setting in disability, aged care, or community and home care.
- Confidence exercising independent clinical judgement and making the call when it matters.
- Outstanding communication skills and the ability to multitask, prioritise and time manage to get to what matters most immediately
- Computer literate and confident with navigating multiple communication platforms and management systems
- Working knowledge of the NDIS including the NDIS Practice Standards and High Intensity Daily Personal Activities requirements, pricing guide and industry processes
- A collaborative style. You will work closely with our Workforce Planner, Care Partners, Operations Manager, Support Staff and our Director daily.
- Current driver's licence and car, NDIS Worker Screening Check (or willingness to obtain), and First Aid/BLS.
