Position Title: Clinical Nurse

On-Call Requirement: Participation in a 1-in-4 after-hours on-call roster

Position Purpose

The Clinical Nurse provides clinical leadership, governance and oversight to ensure the delivery of safe, evidence-based and person-centred care across the organisation's community services.

The role combines direct clinical support with organisational leadership responsibilities including clinical governance, comprehensive assessments, care planning, quality improvement, infection prevention and control, education, workforce development and clinical risk management.

Working closely with the Care Manager, the Clinical Nurse ensures services are coordinated, responsive to client needs, compliant with legislative and organisational requirements, and continually improving in quality and safety. Together, they provide leadership across clinical and operational service delivery to support clients to remain living safely and independently within their own homes.

Key Responsibilities

1. Clinical Advisory Role

The Clinical Nurse will:

- Act as the organisation's primary clinical advisor for nursing and complex care matters.
- Provide expert clinical guidance and support to Registered Nurses, Enrolled Nurses and Support Workers.
- Work collaboratively with the Care Manager to coordinate client services and clinical interventions.
- Provide clinical advice to the Care Manager regarding complex client presentations and changing health conditions.
- Participate in joint case reviews, multidisciplinary meetings and service planning.
- Support the Care Manager in identifying clinical risks and implementing appropriate interventions.
- Support escalation and clinical decision-making for deteriorating clients.
- Liaise with General Practitioners, Nurse Practitioners, Allied Health Professionals, Specialists, Hospitals and external healthcare providers.
- Promote evidence-based practice throughout the organisation.

2. Clinical Governance, Assessments and Care Planning

The Clinical Nurse will:

- Lead and support the organisation's Clinical Governance Framework.
- Complete comprehensive nursing assessments for new and existing clients.
- Develop, implement and regularly review individualised care plans.
- Undertake reassessments to ensure services remain clinically appropriate.
- Monitor clinical outcomes and modify care plans where required.
- Work in partnership with the Care Manager to ensure care plans accurately reflect assessed clinical needs and client goals.
- Collaborate with the Care Manager to review service effectiveness and recommend improvements.
- Ensure all clinical documentation complies with legislative, professional and organisational standards.
- Maintain compliance with professional nursing standards and applicable community care quality standards.

3. Infection Prevention and Control

The Clinical Nurse will:

- Lead the organisation's Infection Prevention and Control Program.




- Develop, implement and review infection control policies and procedures.
- Monitor infection risks across all community services.
- Investigate infection incidents and outbreaks.
- Provide expert advice regarding Personal Protective Equipment (PPE).
- Ensure compliance with Standard and Transmission-Based Precautions.
- Monitor staff competency in infection prevention and hand hygiene.
- Implement quality improvement initiatives relating to infection control.

4. Clinical Education and Training

The Clinical Nurse is responsible for developing and delivering clinical education programs to nursing and care staff, including:

- Medication management
- Continence management
- Diabetes care
- PEG and Enteral Feeding
- Tracheostomy care
- Palliative and End-of-Life Care
- Pressure Injury Prevention
- Infection Prevention and Control
- Clinical Documentation
- Recognition and Escalation of Deteriorating Clients
- Mandatory Clinical Competencies

Additional responsibilities include:

- Developing annual clinical education plans.
- Coordinating competency assessments.
- Mentoring graduate and junior nurses.
- Delivering clinical updates and toolbox education sessions.
- Assisting the Care Manager with the ongoing clinical development of support staff.

5. Clinical Incident Management

The Clinical Nurse will:

- Review all clinical incident reports.
- Investigate adverse clinical events.
- Conduct Root Cause Analysis where appropriate.
- Identify clinical trends and implement preventative strategies.
- Develop corrective action plans.
- Ensure mandatory reporting obligations are met.
- Participate in organisational clinical risk management.
- Present findings and recommendations to senior management.
- Work collaboratively with the Care Manager to improve service delivery following clinical incidents.

6. Recruitment and Workforce Development

The Clinical Nurse will:

- Participate in recruitment and selection of Registered Nurses, Enrolled Nurses and other clinical staff.
- Review applications and participate in interviews.
- Assess clinical competency during recruitment.
- Participate in reference checking.
- Support onboarding and orientation of new clinical staff.
- Conduct probation reviews where required.
- Mentor and support newly employed clinicians.
- Assist the Care Manager in developing workforce capability and succession planning.

7. Monitoring, Continuous Improvement and Clinical Quality

- Clinical audits
- Incident trend analysis
- Clinical Key Performance Indicator (KPI) monitoring
- Client outcome monitoring

Responsibilities include:

- Working closely with the Care Manager to monitor clinical quality indicators.
- Identifying opportunities to improve clinical services.
- Implementing evidence-based practice.




- Monitoring compliance with organisational policies and procedures.
- Participating in accreditation activities.
- Supporting organisational quality improvement projects.
- Monitoring client feedback relating to clinical services.
- Promoting a culture of safety, accountability and continuous improvement.

8. After-Hours Clinical On-Call

The Clinical Nurse will participate in a 1-in-4 on-call roster and provide:

- Telephone clinical support to staff.
- Management of urgent clinical incidents.
- Support with replacement clinical staffing where required.
- Documentation of after-hours clinical advice and actions.

Professional Responsibilities

- Maintain current unrestricted AHPRA registration as a Registered Nurse.
- Practice within professional scope at all times.
- Maintain skilled indemnity requirements.
- Maintain current CPR and First Aid certification.
- Complete all mandatory annual competencies.
- Maintain confidentiality and privacy obligations.
- Comply with organisational policies, procedures and relevant legislation.
- Demonstrate professional leadership and uphold the organisation's values.

Selection Criteria

- Current unrestricted registration as a Registered Nurse with AHPRA.
- Minimum two years post-registration clinical experience.
- Minimum two years' experience within community nursing, aged care, primary health or home care.
- Demonstrated knowledge of Clinical Governance frameworks.
- Strong clinical assessment and care planning skills.
- Experience managing complex clinical presentations.
- Experience investigating clinical incidents.
- Strong leadership, mentoring and coaching skills.
- Excellent written, verbal and interpersonal communication skills.
- Current Driver Licence.
- National Police Check.
- NDIS Worker Screening Check (where applicable).
- Current First Aid and CPR certification.
- Postgraduate qualification in Community Nursing, Gerontology, Primary Health Care, Clinical Leadership or equivalent.
- Experience delivering services within Support at Home, Home Care Packages, DVA Community Nursing and/or NDIS.
- Experience in accreditation and quality systems.
- Infection Prevention and Control qualifications or significant practical experience.
- Experience supervising multidisciplinary teams.

Key Performance Indicators (KPIs)

- 95% of clinical documentation completed within required organisational timeframes.
- 95% of care plans reviewed within scheduled review periods.
- Staff clinical competency compliance maintained at ≥95%.
- Medication audit compliance maintained at ≥95%.
- Infection prevention and control audit compliance maintained at ≥95%.
- 95% of clinical incidents investigated and closed within organisational timeframes.
- 95% compliance with mandatory clinical education and competency requirements.
- Positive trends in client clinical outcomes and reduced preventable clinical incidents.
- Active participation in continuous quality improvement initiatives.
- Positive feedback from clients, families, staff and external stakeholders regarding clinical care and leadership.
