We are looking for an enthusiastic Disability Care Coordinator/Support Coordinator to further develop and grow our business whilst also empowering our broader local community.
Your attention to detail driving the delivery of high customer service excellence with the ability to assist consumer budgets across different packages in either the disability or aged care sector.
Responsibilities
- Assist in-home assessments, referrals, care plan reviews & goal planning for consumers
- Excellent time management with client case load
- Respond & communicate effectively with clients re: changing care needs
- Possess empathy & understanding towards the needs of older people
- Working within a broader team and liaising with our expert staff
- To work within a supportive disability/home care team of dedicated professionals
- Thorough systems & processes in place

Requirements for Success
- Languages spoken - Vietnamese and English
- Essential Vietnamese language ability is required for effective communication with our Vietnamese-speaking client base
- At least 2 years' care management or health care experience
- Knowledge of & enthusiasm for Disability and Aged Care Services & a Consumer Directed care environment
- Diploma level or above qualification in community, aged, health or another discipline desirable
- Access to a reliable, registered motor vehicle with full comprehensive insurance and a valid driver's license
- A transparent national police check and working with children check upon role offer
