Disability Support Worker – Community Access (NDIS)

We are seeking a Support Worker to assist a participant in the community, supporting them to attend appointments, stay active, and maintain independence in everyday life.

Location

Springfield (preferred)

Days Available

Monday to Sunday (flexible roster)

Applicants located in or near Springfield are strongly encouraged to apply due to participant preference.

About the Role

This position involves supporting participants both at home and in the community. Activities may include attending appointments, shopping, community outings, and assisting with everyday routines.

Key Responsibilities

- Supporting participants to attend appointments and community activities
- Assisting with shopping, errands and everyday tasks




- Encouraging independence and community participation
- Building respectful relationships with participants
- Maintaining explicit progress notes and communication with the team
- Experience working in disability support or a similar role
- Current Driver’s Licence and reliable vehicle
- NDIS Worker Screening Check
- Current First Aid and CPR
- Ability to work independently and maintain clear documentation
