Remuneration:$79,419.00 - $122,465.00 per annum plus superannuation for Full Time

What you'll be doing

South Eastern Sydney Local Health District (SESLHD) is committed to improving the care provided to our patients in line with our vision of Working together to improve the health and wellbeing of our community.

The clinical assessor undertakes a complex aged care assessment using the Integrated Assessment Tool (IAT) addressing the medical, physical, cognitive, psychosocial, cultural, environmental and social aspects of older people referred for assessment via the My Aged CareContact Centre. The clinical assessor formulates a support plan that best meets the needs of the older person and their carer within the resources available. The clinical assessor works within the guidelines set by the Single Aged Care System Assessment Services Contract Statement of requirements, the My Aged Care Assessment Manual 2024 and the Aged Care Act 1997.

Selection Criteria





- Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award, Public Health System Nurses and Midwives (State) Award or for psychologist, a recognised 4 year degree in psychology in line with the NSW Health and Community Employees’ Psychologists (State) Award 2024 and current registration with a relevant Registration Board (AHPRA), or membership or eligible for membership of relevant recognised profession body and demonstrated commitment to ongoing professional learning.
- Minimum of one year demonstrated experience in Australia or overseas directly delivering services in aged care settings and/or to aged persons (such as geriatric evaluation, rehabilitation, palliative care, community nursing), including to people living with dementia.
- Demonstrated high level written, verbal and interpersonal skills, to both engage patients/clients and work effectively with the multi-disciplinary team.
- Demonstrated ability to proactively engage with patients/clients to enhance service delivery and to guide others in providing service excellence.
- Demonstrated ability to work as part of a multi-disciplinary team and apply professional and ethical boundaries when dealing with complex situations.
- Proficient skills in professional and clinical digital technologies i.e. eMR, CHOC, My Aged Care, Microsoft Office applications.
- Detailed knowledge and understanding of risk management, safety and quality principles and the role of the Clinical Assessor in applying these principles.




- Current drivers' licence and willingness to travel in accordance with the demands of the position.
