Join North and West Remote Health (NWRH) as a Care Partner and play a meaningful role in supporting older people across Mount Isa. Working closely with clients, their families, carers, and service providers, you'll coordinate personalised care and connect people with the services they need to achieve their goals and maintain their independence.

- Salary $68 969 + Super + Salary Packaging;
- Salary sacrificing options up to $15,900 to help maximise your take home pay;

About the role

- Conduct comprehensive client and home assessments to develop and review personalised care plans.
- Coordinate and monitor aged care supports, ensuring services meet clients' changing needs and goals.
- Guide clients and families through the aged care system, helping them access appropriate services and make informed decisions.
- Collaborate with health professionals and service providers to coordinate care and facilitate referrals when required.
- Maintain accurate records, build strong relationships with clients and stakeholders, and participate in outreach services across regional and remote communities.

Skills and experience

- Demonstrated experience in community aged care, case management, care coordination, or related support services
- Knowledge of community aged care programs and funding models
- Understanding of person-centred and community-based service delivery
- Excellent written communication and administrative skills
- Willingness and ability to undertake regular outreach travel, including overnight stays and travel by road and small aircraft.
