Conder Surgery is seeking an experienced and compassionate Social Worker to join our friendly and supportive multidisciplinary team. This position offers the opportunity to work closely with GPs and other health professionals to provide holistic care that addresses both the medical and social factors affecting health and wellbeing.
About the Role
This is a part-time position (19 hours per week, negotiable) with flexibility around days and hours. The role would suit an experienced social worker looking for work-life balance or someone seeking to expand their experience within primary healthcare.
You will support patients experiencing a range of challenges, including:
- Mental health concerns
- Chronic health conditions
- Family and relationship issues
- Social isolation
- Housing and financial hardship
- Grief and loss
- Domestic and family violence
- Navigating health, disability, aged care and community support systems
Key Responsibilities
- Undertake psychosocial assessments and care planning.
- Provide brief interventions, counselling and emotional support.
- Assist patients to access relevant community, health and social services.
- Support care coordination for patients with complex needs.
- Work collaboratively with GPs, nurses and allied health practitioners.
- Participate in multidisciplinary case discussions and care planning.
- Maintain accurate and timely clinical documentation.
Essential Criteria
- Bachelor or Master's degree in Social Work.
- Eligibility for membership with the Australian Association of Social Workers (AASW).
- Demonstrated experience in social work practice.
- Strong communication, assessment and advocacy skills.
- Ability to work independently while contributing to a multidisciplinary team.
- Current Working with Vulnerable People registration (or willingness to obtain).
- Current National Police Check.
Desirable
- Experience in general practice, primary healthcare, community health or mental health settings.
- Knowledge of ACT community support services.
- Experience supporting patients with chronic disease, disability, mental health or aged care needs.
