Wayside Chapel is seeking a Care Coordinator to work alongside our team to empower visitors to achieve personal goals. You will understand homelessness and related mental health and AOD issues with compassionate, evidence‑based case management.

You will build rapport, assess needs, develop strengths‑based care plans, coordinate with health, housing and legal services, and support frontline staff to promote independence and wellbeing in a trauma‑informed framework.
