You'll tailor supports to their goals, vibes, routines, values, and culture. You'll team up with families, medical professionals, and other support workers to keep everything running smoothly. Shifts flexible with your day, and you'll build genuine relationships, not just checklists

What you'll do

- Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
- Medical supports : Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
- Social time and participation : Bring meaning, fun, and community access aligned with interests.
- Autonomy and advocacy : Support decision-making, respect consent, boost independence where possible.




- Cultural humility : Respect backgrounds, beliefs, and ways of being

Who we're looking for

A person-centered , warm, patient, and genuinely caring soul. Interest or experience in complex care , disability, and health nuances.

- Reliable and qualified , with good notetaking and teamwork.
- Flexible and adaptable ; comfortable with changing needs and shifts.
