NSW Health at Blacktown Hospital is seeking an Aboriginal Health Worker to provide advocacy and culturally secure support for Aboriginal patients and their families throughout the hospital journey.

You will work with a multidisciplinary team to assist discharge planning, coordinate referrals, and connect patients to community services, contributing to Aboriginal health programs and education.
