The Registered Nurse / Diabetes Educator is responsible for delivering high-quality clinical nursing care, diabetes education, health promotion, chronic disease management, and participant support services across aged care, disability, community, and outreach settings.
The role focuses on improving health outcomes, self-management capacity, and quality of life for individuals living with diabetes and other chronic health conditions. The position involves providing evidence-based education, clinical assessments, care coordination, and person-centred support to participants, families, carers, and support workers.

The Registered Nurse / Diabetes Educator works collaboratively with multidisciplinary teams, medical practitioners, allied health professionals, community organisations, and government stakeholders to ensure safe, effective, and culturally appropriate healthcare delivery across regional and remote communities.

Key Responsibilities

Clinical Nursing Practice

- Conduct comprehensive health assessments and ongoing clinical reviews.
- Develop, implement, and evaluate individualised care plans.
- Monitor and manage participants with chronic health conditions, including diabetes.
- Administer medications and perform clinical procedures within scope of practice.
- Identify and respond to changes in participant health status.
- Provide wound care, health monitoring, and clinical support services.
- Coordinate referrals and liaise with healthcare providers as required.

Diabetes Education and Chronic Disease Management

- Provide evidence-based diabetes education to individuals, families, carers, and support workers.
- Support participants in understanding diabetes management, medication compliance, nutrition, exercise, and lifestyle modifications.
- Assist participants to develop self-management skills and improve health literacy.
- Monitor participant progress and assist in achieving clinical and health-related goals.
- Facilitate education sessions and community health promotion activities.




- Support early intervention and prevention strategies for diabetes-related complications.

Participant and Community Support

- Promote participant independence, wellbeing, and quality of life.
- Deliver person-centred and culturally responsive care.
- Support participants from diverse cultural, linguistic, Aboriginal and Torres Strait Islander, and vulnerable community backgrounds.
- Build positive relationships with participants, families, carers, and community stakeholders.
- Participate in outreach programs and community-based health initiatives.

Documentation and Reporting

- Maintain accurate clinical records, assessments, progress notes, and reports.
- Ensure all documentation complies with legislative, professional, and organisational requirements.
- Participate in case conferences, multidisciplinary meetings, and care planning activities.
- Report incidents, risks, safeguarding concerns, and clinical issues in accordance with organisational policies.

Compliance and Quality

- Practice in accordance with AHPRA Nursing Standards, Nursing and Midwifery Board of Australia requirements, and relevant legislation.
- Maintain compliance with NDIS Practice Standards, Aged Care Quality Standards, and workplace policies.
- Uphold participant confidentiality, privacy, and dignity at all times.
- Contribute to continuous quality improvement activities.
- Current registration as a Registered Nurse with AHPRA.
- Credentialled Diabetes Educator (CDE) or working towards credentialling, or demonstrated experience in diabetes education and chronic disease management.
- Minimum two years' post-registration nursing experience.
- Experience in primary health care, community nursing, aged care, disability services, chronic disease management, or related settings.




- Strong clinical assessment and care planning skills.
- Excellent communication and interpersonal skills.
- Current First Aid and CPR Certificate. (ability to obtain prior to commencement of employment)
- Current Australian Driver Licence/International License
- NDIS Worker Screening Clearance. (ability to obtain prior to commencement of employment)
- Working with Children Check (ability to obtain prior to commencement of employment)
- Ability and willingness to travel throughout regional and remote communities.

Desirable Skills and Experience

- Experience working with Aboriginal and Torres Strait Islander communities.
- Experience delivering outreach and community-based healthcare services.
- Knowledge of diabetes technologies including continuous glucose monitoring and insulin management.
- Understanding of NDIS, aged care, and community health systems.
- Experience supporting participants with complex health needs and multiple comorbidities.

- Strong health education and coaching skills.

Working Conditions

- Community visits and outreach service delivery.
- Travel throughout Far North Queensland and remote communities.
- Independent service delivery in regional and remote locations.
- Clinical assessments, participant support, and health education activities.
